We encourage you to fax applications to our designated fax number: 1-877-229-5533. The initial premium may be credited through an Automatic Bank Withdrawal. The withdrawal date will be the issue date if a date is not specified.

Agent Contracting
Due to the volume of new agent contracts currently being received, we are experiencing a delay in processing agent appointments. Steps have been taken to address our backlog and improve our turnaround time. Agent contracts received with an application are given priority.

Assurity Non-Resident Appointment Information

Assurity will pay the resident appointment fee as long as the agent was not previously terminated for insufficient production with Assurity.
Non-resident fees are the agent's responsibility.
